KVKK Application Form

In accordance with the Communiqué on the Procedures and Principles for Applications to the Data Controller, applications can be made through the following channels:

• By a petition bearing your wet signature, in person or through a notary: Hacıkaymak Mah. Zuhuri Mehmet Sk. 1.2. Girişler No: 7 İç Kapı No: 10, Selçuklu / Konya
• By email signed with a secure electronic signature or mobile signature, or from the email address you previously provided to Kullya and that is registered in our system: info@kullya.com

For applications where we cannot verify your identity, we may request additional verification to prevent disclosing someone else's data. For this reason, emails sent from an address other than your registered email address are not sufficient on their own.

• Your first name, last name and, if the application is in writing, your signature,
• if you are a Turkish citizen, your Turkish national identification number; if you are a foreign national, your nationality and passport or identity number,
• your residential or business address for service of notices,
• your email address for notifications, and your phone and fax number (if any),
• the subject of your request: which right you wish to exercise and an explanation of your request,
• your relationship with Kullya (member, customer, visitor, visitor of a Kullya website, etc.) and, if applicable, the email address or organization name registered to your account,
• information and documents supporting your request.

If you are applying on behalf of someone else, you must attach a notarized power of attorney or a legal representation document showing that you are authorized to do so.

You have the right to learn whether your personal data is processed, to request information if it has been processed, to learn the purpose of processing and whether it is used in line with that purpose, to know the third parties to whom it is transferred, to request correction, deletion or destruction, to request that these actions be notified to the third parties to whom the data was transferred, to object to an outcome against you resulting from automated analysis, and to claim compensation for damage. Your application will be concluded free of charge as soon as possible depending on the nature of the request, and within 30 days at the latest. The response is sent in writing or electronically, according to the preference stated in your application. If the process requires an additional cost, a fee may be charged according to the tariff set by the Personal Data Protection Board; if the application arises from an error by Kullya, the fee charged is refunded.

If your application is rejected, if you find the response insufficient, or if no response is given in time, you may file a complaint with the Personal Data Protection Board within 30 days of learning of the response and, in any case, within 60 days of the application date.

For data on Kullya customers' websites, the data controller is the relevant website owner. Apply to that website first. If you cannot reach the website owner, write to us and state the website's address in your application; we will forward your request to the website owner and keep you informed about the process.
